Avory Home Care Solutions LLC (“Avory”) is committed to protecting the confidentiality, integrity, and security of Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable federal and state privacy laws.
Avory operates as a Business Associate to covered entities and executes a Business Associate Agreement (BAA) prior to accessing, receiving, or reviewing any PHI.
Use and Disclosure of PHI
PHI accessed by Avory is used solely for the purpose of performing contracted clinical quality services, including but not limited to:
OASIS review and validation
ICD-10-CM coding review and alignment
Clinical documentation improvement (CDI)
Quality assurance and compliance feedback
PHI is accessed only to the minimum extent necessary to fulfill contractual obligations and is not disclosed to third parties except as permitted by HIPAA, required by law, or authorized in writing by the client agency.
Safeguards and Security Measures
Avory maintains administrative, technical, and physical safeguards designed to protect PHI, including:
Secure file transfer and/or controlled EMR-based access
Role-based access controls
Encrypted communications where applicable
Workforce HIPAA training and confidentiality agreements
Access to PHI is limited to authorized personnel with appropriate training and a legitimate business need.
Data Retention and Disposal
PHI is retained only for the duration necessary to fulfill contractual and regulatory requirements. Upon completion of services or termination of an agreement, PHI is securely returned or destroyed in accordance with HIPAA standards and the terms of the applicable BAA.
Compliance Commitment
Avory regularly reviews and updates its privacy and security practices to maintain compliance with HIPAA and applicable regulatory requirements. Any suspected or confirmed privacy incidents are handled in accordance with HIPAA breach notification standards and contractual obligations.
